Calvary requires that staff of all suppliers, including any subcontractors, who are accessing and providing services to our facilities (hospital, aged care and community care) will need to provide proof to us of their COVID-19 vaccination. In accordance with the various State and Federal directives and revisions of our internal policies staff of suppliers accessing our facilities will be required to have received their first dose of Covid19 vaccination by the following dates in order to be permitted access to our sites:
As a supplier to Calvary this is a mandatory condition of entry to our facilities. How do I provide my COVID-19 vaccination statement to Calvary? You would have received previous communications regarding registering to our online portal, Calvary Supplier Central. This is the online platform which Calvary is using to collect compliance documents from suppliers, such as insurance certificates of currency and COVID-19 vaccination statements. During July and August, we have been progressively on boarding suppliers to Calvary Supplier Central, and will continue to do so over the coming weeks. Once on boarded you will be required to provide critical documentation including records of COVID-19 vaccination. If you have not received your login details to Calvary Supplier Central, please notify us by sending an email to nationalprocurement@calvarycare.org.au so that we can register you for access to the portal. Please ensure that the email address do.not.reply@decisionmax.com.au is whitelisted so that you will be able to receive login details and portal notifications such as upcoming expiry dates of certificates.
