Dear Community, Happy fall to all! We hope you are adjusting to the variety of new and creative school schedules and colder weather. We are excited to share plans for a virtual ACTION meeting, slated for October 29th and 30th (save the date info below). Agenda details are still being worked out, but we will share more soon. There will be a general session as well as virtual breakouts, and we hope to cover topics such as DMD, heart failure, patient/caregiver education as well as other exciting topics. Since we will be hosting this meeting virtually, we will share information on how to register for specific breakouts and sessions. Some portions of the meeting will be open to all ACTION members, and some sessions will be reserved for specific committees and/or groups in order to facilitate virtual work in smaller, more focused groups. We will not have an ACTION webinar this month due to the planning for the virtual meeting. Stay tuned for future webinars coming soon! Quality Improvement UpdatesCOVID-19 CallsThank you to those who continue to share learnings and experiences on the COVID-19 calls with ACTION and PHTS! These calls are now monthly, with the next call on October 22, 2020. Discharge CommitteeThe next Discharge Committee meeting will take place on October 7, 2020. Site specific data was sent out in Sept. to the project key contacts. Please review your data to ensure it is accurate. Contact Paige Krack if you did not receive your data or have questions. Please continue to test the Flight Plan and Journey Map, and enter data into REDCap on any patients who have been discharged or transferred to the floor. Stroke QI ProjectWe believe that we can drive down strokes even further and are working on re-launching ABCs 2.0 in a few months. ABCs 2.0 will also include reducing bleeding events. Please continue to enter data into REDCap for your VAD patients in the meantime. Telehealth Project22 participating teams are testing an algorithm around defining which patients qualify for virtual visits and sharing education materials for care teams and patients/families. We are diving into measures so we can accurately capture the quality of telehealth visits. The next huddle is on October 14, 2020 . Please continue to enter your data into REDCap for the provider and patient surveys. MOCACTION is now approved for ABP MOC Part 4 credit! The operations team is working on a process to help physicians in ACTION claim MOC credit, and we will be communicating the plans and process soon. Stay tuned! Fellows in ACTIONThe Fellows working group within ACTION had our first meeting for this academic year on September 29th, and we are looking forward to working and learning together!
