Bringing Patient-Centered Care to Patients with Alcohol Use Disorders Journal of the American Medical Association Current medical management of alcohol use disorder (AUD) is usually in stark contrast to concepts of patient-centered care. Shared decision making for AUD can be integrated into primary care. Primary care clinicians should assess AUD severity, medical and psychiatric comorbidities, reasons patients may or may not want to change their drinking, and whether they want help doing so. Clinicians and patients should discuss ambivalence toward change; patient goals (eg, abstinence vs decreasing drinking vs no change); preference for group-based, one-on-one, or medication treatments or some combination...

The Transition to Medication Adoption in Publicly Funded Substance Use Disorder Treatment Programs Journal of Studies on Alcohol and Drugs Medications for the treatment of substance use disorders (SUDs) are not widely available in publicly funded SUD treatment programs. Few studies have drawn on longitudinal data to examine the organizational characteristics associated with programs transitioning from not delivering any pharmacotherapy to adopting at least one SUD medication. Adoption of pharmacotherapy was more likely in programs that had greater medical resources, Medicaid funding, and contact with pharmaceutical companies. Given the potential expansion of Medicaid under the Affordable Care Act, patients served by publicly funded programs may gain greater access to such treatments...

Early Life Trauma and Directional Brain Connectivity Within Major Depression Human Brain Mapping Early life trauma (ELT) is a significant risk factor for the onset of depression. Emerging findings indicate ELT is associated with enhanced amygdala reactivity to aversive stimuli in never-depressed healthy controls as well as those with acute depression but may be absent in non-ELT exposed depressed. The precise mechanism mediating these differences in amygdala reactivity remains unclear.
