Pick five caregivers who assist with transfers—any shift, any role.

Ask them:

  • How do you know how this resident should be transferred?
  • Where do you find that information?

  • If you were floated to a new hall tomorrow, how would you figure it out?

 

Now pay attention.

If you get hesitation, inconsistency, conflicting answers, or confusion—that’s your red flag.

Not because they’re failing.

But because your system is.

  • Retrain on transfer methods and where to find that information.
  • Reinforce who to ask and how to escalate when unsure.
  • Re-audit in 30 days. Don’t let it drift.

Because the scariest part of a transfer isn’t a missed policy—it’s a life-changing outcome.
