Student Vaccination and Booster Clinic This message provides information about COVID vaccinations for students who are 5 years of age and older. Dear Parents/Guardians and Families, Lyons Township High School District #204 is excited to host a vaccination event for all individuals who are eligible to receive the COVID-19 vaccination or booster. This includes individuals aged 5-11, per the latest CDC/FDA approval granted on Monday, November 1. This vaccination event is hosted in cooperation with Albertsons/Jewel-Osco and all LT Associate School Districts. By creating more opportunities to boost the rate of vaccinated people in our area, our hope is that it will lead to a lower number of new infections, hospitalizations and create safer, healthier schools for all our students. Any individual, 5 years of age and older, living in the Lyons Township High School District #204 community or within any of LT’s Associate School communities, can register to receive a free Pfizer or Moderna COVID-19 vaccine at LT's South Campus Fieldhouse on Saturday, November 13 (1st dose) and Saturday, December 4 (2nd dose). In addition, COVID-19 Pfizer or Moderna boosters are available for those in the recommended groups. For those receiving the vaccination for the first time, we will provide an opportunity to upload the vaccination card after the second dose. Two Steps are Required
For those registering for the Pfizer COVID-19 vaccine, your December 4 appointment time will be the same as the time you schedule for November 13. On the Day of Your Appointment Please bring the following to your vaccination appointment:
Clinic participants should park on the north side of LT’s South Campus (closest to Bennet Field) and enter the Fieldhouse entrance. LTHS South Campus is located at 4900 S. Willow Springs Rd., Western Springs. Please note that masks and social distancing are required at the vaccination clinic. Vaccine Clinic Volunteers Needed Please consider volunteering for the Saturday, November 13, community-wide COVID-19 clinic. Volunteers will help check-in clinic registrants, make copies of required paperwork, help direct people to appropriate vaccination stations, and assist vaccination personnel. No medical training is necessary. Please consider volunteering for a morning (7:45-11:30), afternoon 11:30-3:00), or all-day shift.
