Self-Attestation Questionnaire
New Requirements
Beginning April 24, 2023, the Centers for Medicare and Medicaid Services (CMS) will require that researchers:
- Use the new Research Identifiable File (RIF) request forms for all Data Use Agreement (DUA) actions.
- Have an approved Data Management Plan Self-Attestation Questionnaire (DMP SAQ) for a new DUA request or extensions of an existing DUA with physically shipped data.
If your DUA includes physically shipped data and you do not have an approved DMP SAQ, you will risk losing access indefinitely to CMS physically shipped data.

How to complete fields on CMS DUAs
Item 1: The agreement is with “University of Iowa”
Signature Block: Requestor Name is “Wendy Beaver”
Signature Block: Data Custodian Name is the PI’s name or other UI data custodian
After receiving approval from CMS/ResDAC, route the DUA request to the Division of Sponsored Programs on a Non-Monetary Routing Form. The DUA should not be signed prior to receiving CMS/ResDAC approval.
