Practice managers, have you joined these peer networks?
SEMPHN is committed to providing support to staff working in primary care, and supporting and establishing local professional networks for practice managers. The SEMPHN Practice Network Meetings are held every second month. Each meeting presents a different topic and key speaker and all participants receive a Statement of Attendance. Participants also have the opportunity for informal networking with other practice and SEMPHN staff during these meetings. Register for the upcoming Practice Network here. SEMPHN offers an online platform called Basecamp to enable practices to stay connected and share information and resources with their peers. Practice Managers in the catchment can join Basecamp by requesting it either through their Provider Support Officer or by contacting SEMPHN. Email: providersupport@semphn.org.au

Managing COVID-19 positive
patients in general practice
With rapidly increasing COVID-19 cases in Melbourne, the need to care for COVID-19 positive patients in the community who are deemed 'low risk', is growing fast. While GPs will manage their existing patients, we are also asking GPs to take on new COVID-positive patients to manage their care from home. Existing patients: Please familiarise yourself with the COVID-Positive Pathway - a partnership between local hospital networks, community health and primary care. Every person who tests
positive is offered enrolment in the Pathway, so your practice will receive calls from the Pathway team with regards to your patients' transfer of care. New patients: Please complete this brief form to express your interest in taking new COVID-positive patients. If you have already done this previously, you do not need to do this again. Webinar recording: Managing COVID-Positive Patients in the Community using the Pathway Learn how to best
manage your COVID positive patients living in the community and how to identify signs of deterioration and escalate your patient’s care when necessary. View the recording. Passcode: *.0gqSN^

Seeing patients with hepatitis C? You may receive a call from this new
pilot program
The Victorian Department of Health and the Doherty Institute have partnered in a pilot program to actively follow up notifications of chronic hepatitis C to raise awareness of the newly available treatment pathways and improve the coordinated delivery of care to Victorians living with chronic hepatitis C. The Coordinated Hepatitis responses to Enhance the Cascade of Care by optimising existing Surveillance systems (CHECCS) program will support diagnosing clinicians by providing resources and guidance for the management and treatment of hepatitis C, including appropriate referral pathways. If you are a diagnosing clinician of a patient with hepatitis
C, you may receive a call from the Victorian Department of Health seeking further information about test results and the availability of treatment options for your patient. CHECCS has been funded by the Eliminate Hepatitis C Australia partnership as part of their national mission to establish coordinated responses to eliminate hepatitis C as a major public health threat by 2030. For more information about the pilot program, contact Mielle Abbott, Health Protection Branch mielle.abbott@health.vic.gov.au

Consultation open: Provide feedback on the Disability Act
The Victorian Government has published the Review of the Disability Act 2006 consultation paper online following public consultations held earlier in the year and work by the Disability Act Review Advisory Group. Feedback from the community will help ensure the Disability Act appropriately reflects Victoria’s role post-NDIS transition and continues to promote and protect the rights of people with disability, supports their needs and aspirations, and advances their inclusion and participation on an equal basis with others. The consultation runs for four weeks until 20 October 2021. For more information and to have your say, visit engage.vic.gov.au/disability-act.
Ageism Awareness Day (1 October)
Ageism Awareness Day on 1 October is an opportunity to draw attention to the existence and impacts of ageism in Australia, which is a critical step to changing community attitudes. Ageism can impact health and pathways to support. To get involved for Ageism Awareness Day, you can access the campaign resources which include social media posts, posters, flyers and even a conversation guide. View the resources here.
