This is Lawndale Health – Fitness | Nutrition | Care

LCHC respects the cultural diversity of Chicago and is aware of the concerns and uneasiness our migrant communities may be experiencing due to the new administration.

We want to reassure our communities of our commitment to providing quality, affordable health care. We would like to remind our patients that LCHC does not ask for documentation status in patient registration processes in order to provide care. Further, we are committed to protecting your personal health information. For more details, please review our privacy policies on our webpage.

We want all our patients to know that they are safe when receiving care here. We also encourage our communities to know their rights and seek support safely and accordingly.

Please review and share the following resources in your community:

Get Help to Quit Smoking

Smoking is the leading cause of preventable death, responsible for over 480,000 deaths in the U.S. annually. This dangerous habit harms nearly every organ in the body, significantly increasing the risk of heart disease, stroke, lung disease, and cancer. Quitting smoking not only reduces these health risks but can also extend your life expectancy by up to 10 years. While the journey to quit smoking can be challenging, support from your care team, loved ones, and community resources makes a big difference. Take the first step toward a healthier future by reaching out to your care team for personalized guidance and tools or call the Tobacco Quitline to help you quit for good.

The Illinois Tobacco Quitline is a great and free resource for people who want to quit smoking! Call 1-800-784-8937.

Get to Know Our Providers

Rachel Maibach, DO - Family Medicine Resident

Rachel Maibach, DO, first encountered LCHC during her second year of medical school at a Christian Community Health Fellowship conference. Despite initially planning to stay close to her Indiana roots for residency, her perspective shifted dramatically after an inspiring interview at Lawndale, where the mission of proximity and service to the community deeply resonated with her. A graduate of A.T. Still University in Missouri, she brings a holistic approach to patient care, addressing not only physical health but also mental, spiritual, and social factors. Dr. Maibach shares, "One of my strengths is creating a safe space for patients where they feel comfortable to share what’s going on." Dr. Maibach values clear communication and building trust through patient-centered care. Guided by her faith, she finds immense reward in witnessing the positive impact of her work, whether through physical healing or offering spiritual and emotional support.

She says, “At Lawndale, we look beyond physical health—we care for the whole person: their mental, spiritual, and social wellbeing too.”
