CASE OF THE MONTHMonthly Case from the Department of Medicine Submitted by: Aurangzeb Memon, MD Edited by: STORY AND CASE: A 32-year-old man with a past medical history of hypertension and stage IV chronic kidney disease (CKD) was brought to the hospital by emergency medical services after he was found unresponsive. His initial blood pressure was 266/166 mmHg in the ambulance. Upon physical examination, the patient remained unresponsive, with occasional gaze deviation to the right. He was unable to follow commands. Computed tomography (CT) and CT angiography (CTA) of his head were performed due to concern for possible stroke, with no evidence of hemorrhage or large vessel occlusion seen. He had elevated troponins as well as possible ischemic changes on electrocardiogram, both of which were believed to be due to demand ischemia from severe hypertension. He was admitted to the intensive care unit (ICU) with acute on chronic kidney disease requiring emergent dialysis. He exhibited occasional head nodding, with electroencephalogram being negative for epileptiform discharges. Uremic encephalopathy was on the differential, and the patient was initiated on hemodialysis. Magnetic resonance imaging (MRI) of his brain with and without contrast showed pontine enlargement with diffuse T2 hyperintensity throughout the pons, with no evidence of restricted diffusion on diffusion-weighted imaging/apparent diffusion coefficient. We're proud to share that DOM faculty member and Associate Program Director for the IM Residency Program, Jennifer Spicer, MD, MPH, was named among these outstanding young alumni. Dr. Spicer is a clinician educator whose dedication lies in serving both patients and learners. Her journey into the realm of infectious diseases began during her time as a student at Emory School of Medicine in 2007. She then went on to pursue a master's degree at Rollins School of Public Health and completed her medical training at Emory including a residency in internal medicine, and a fellowship in infectious diseases. Dr. Spicer's clinical practice is based at Grady Memorial Hospital, and her most significant source of satisfaction comes from educating future healthcare professionals. She plays a pivotal role as an associate program director for the Internal Medicine Residency Program and holds multiple educational leadership positions working with medical students, residents, and fellows. Division of Pulmonary, Allergy, Critical Care, and Sleep Medicine The Division of Pulmonary, Allergy, Critical Care, and Sleep Medicine recently hosted the 24th annual Roland H. Ingram, Jr. Lecture, gathering many of their faculty, fellows, and staff together in person. Speaker, Sally Wenzel, MD (Chair, Department of Occupational & Environmental Health, Professor of Medicine and Immunology and Director, University of Pittsburgh Asthma Institute at UPMC/UPSOM, Subsection Chief of Allergy, Division of Pulmonary, Allergy & Critical Care Medicine, and UPMC Chair of Translational Airway Biology) delivered an outstanding lecture that showcased her breadth of expertise spanning epidemiology to foundational disease biology to environmental health to drug discovery for novel therapeutics. The evening also included Division Awards, celebrating the many achievements and accomplishments of their faculty and fellows. Congratulations to the following honorees!
DOM faculty selected as fellows for the Woodruff Leadership Academy Class of 2024 Congratulations to Jesse Jacob, MD (Infectious Diseases), Colleen Kelley, MD, MPH (Infectious Diseases), and Jeanie Park, MD, MS (Renal Medicine), who have been named 2024 Woodruff Leadership Academy fellows!
