"Can you die from chest pain 'treatment'?"

Story and Case:

A 40-year-old African American male with past medical history of coronary artery disease (CAD) s/p left anterior descending artery (LAD) stent several months prior to presentation, hypertension, hyperlipidemia, paroxysmal atrial fibrillation, sick sinus syndrome s/p permanent pacemaker (PPM), pulmonary embolism and cerebral vascular accident, presented with severe substernal chest pain. Nitroglycerin (NTG) and isosorbide mononitrate were ordered upon admission. These medicines did not provide any relief of chest pain, and the patient started to request hydromorphone constantly. Of note, patient had experienced recurrent chest pains after initial stent implantation and required two left heart catheterizations (LHC) that showed a patent stent and open coronaries.

As his angina did not resolve, Cardiology performed a functional LHC.  The LHC showed increased cardiac nociception with contrast injection and adenosine infusion, normal non-endothelial pathway microvascular function, and endothelial dysfunction with notable spasm and myocardial muscle bridge of the LAD, worsening with NTG.

Let's Talk Promotions

This month we are sharing feedback from Dr. Vaishali Patel, Division of Digestive Diseases. Dr. Patel was recently promoted to Associate Professor, and we wanted to know what advice she would share with her fellow faculty on the promotion process.

Dr. Patel said, "Start [your] packet early. It's more work than one may expect. Do not miss the meetings that go over all that is needed for the packet. The meetings are very useful, and ours were on Zoom. Stay in communication with the coordinator [who] is helping you with your packet. They are awesome!"

Diversity, Equity, and Inclusion

Congratulations to the recipients of the 2022 American Association of Medical Colleges Group on Diversity and Inclusion (AAMC GDI) Exemplary Leadership Award!

The AAMC GDI award ceremony will take place in November and will recognize the team from Emory and Emory Grady, including Dr. Jada Bussey-Jones and Dr. Kimberly Manning.
Dr. Bussey-Jones has nationally recognized educational expertise in minority health, health equity, as well as patient and provider education. She developed several program initiatives addressing health promotion and disease prevention for vulnerable populations.
Dr. Manning has a passion for storytelling with the goal of building and strengthening diverse and humanistic clinical environments, as well as cultivating psychologically safe learning climates.

Health Equity Advocacy Training Inaugural Cohort

Last month we mentioned the Emory at Grady Health Equity and Advocacy Course. This month we want to say 'Congratulations!' to the twenty-one participants who were selected to participate in this inaugural course, which is a 5-month multi-pronged educational intervention that brings faculty, learners, and community members from several institutions together to advocate around common health-related priorities. The course goes beyond didactic instruction and includes experiential learning, leveraging collaboration and providing tangible outcomes. Participants learn while doing. Connect and Chat Sessions
Did you know the Faculty Staff Assistance Program (FSAP) offers a weekly "Connect & Chat" session to help you process the emotional impact of ongoing societal stressors and traumatic events with others? These sessions will provide a safe space to unpack, share and discuss healthy strategies for coping while navigating life’s daily pressures. The sessions are offered on Tuesdays via Zoom.

 

Refresh and Mindfulness Meditation
Every first and third Wednesday from 4:30–4:45 p.m., you can join the FSAP group for a 15-minute virtual self-care experience to support your resilience. Visit the FSAP Events and Workshop page to register. || On the fourth Wednesday of each month at 1 p.m., you can join Mindfulness Meditation.
