This approach is designed to close gaps in screening completion by reaching both providers and patients directly, supporting better identification of social needs and improving access to appropriate services. One Brooklyn Health (OBH) Engages Healthix to Provide Population Health InsightsOne Brooklyn Health (OBH), under the leadership of President and Chief Transformation Officer Arthur Gianelli, has engaged Healthix to deliver one-time, custom de-identified aggregate reports supporting population health and care coordination efforts.
Leveraging the existing Epic EHR integration between OBH and Healthix, the reports will provide counts of unique patients, encounter types, and common diagnoses for patients seen at selected FQHCs and OBH hospitals. The analysis includes lifetime FQHC patient counts, 2025 encounter activity, and insights into patients receiving care across both settings. A companion report will focus specifically on patients residing in Brooklyn ZIP codes to support borough-level community health analysis.
