CASE OF THE MONTHMonthly Case from the Department of Medicine: Submitted by: Ashley Urrutia, MD, MPH (Left: Shae Chambers; Right: Ashely Urrutia) STORY AND CASE She described the RUQ pain as dull and non-radiating, first occurring after eating a fatty meal 3 days prior. She endorsed a one-month history of bilateral, pruritic rash on the upper extremities as well as bilateral wrist pain for the past 3 months and bilateral knee pain for the past year. She moved one year ago from Peru. She is married and lives with her husband and niece. She denied sick contacts or new sexual partners. Family history was positive for arthritis and “thyroid problems” on the maternal side. She noted a recent history of receiving a contraceptive injection at a beauty shop 1 month ago, but otherwise did not take any medications or supplements regularly. Physical exam was notable for severe RUQ tenderness to palpation, cervical lymphadenopathy, as well as dusky, flat, non-blanchable lesions on the posterior surfaces of upper arms bilaterally ranging from 8mm to 12mm with some excoriations but no skin breakage or ulceration. Work-up on admission showed transaminitis with elevated AST and ALT values of 115 and 136 respectively, as well as an elevated alkaline phosphatase of 421 and elevated gamma-glutamyl transpeptidase of 264. Her HIV, RPR, and hepatitis serologies were all negative. Her initial rheumatologic work-up showed an ANA titer of 1:1280. Urine studies revealed proteinuria and hematuria. A right upper quadrant ultrasound showed mild dilation of the proximal common bile duct with no visualized biliary lesions. A CT of the abdomen/pelvis without contrast showed multifocal cervical, supraclavicular, axillary, mediastinal, and retroperitoneal lymphadenopathy. An x-ray of the bilateral wrists showed subchondral cystic changes involving the ulnar aspect of the lunate bilaterally. Have an interesting case that you want to share with the DOM faculty? Nominations open for 2024–2025 Clinical Distinctions Program The Clinical Distinctions (CD) designation program is an annual School of Medicine program designed to recognize faculty who have devoted a majority of their time and careers to providing excellence in clinical care. Distinctions are awarded at three levels: Senior Physician, Distinguished Physician, and Eminent Physician. Program guidelines, distinction criteria, and required supporting documents may be found here. Please contact your Division Director or Division Administrator if you are interested in being nominated. Divisions must submit names of nominees in each category to the Office of Faculty Development (OFD) by Friday, July 12. As part of your nomination, you will need an updated CV in the proper SOM template. The Teaching Intentionally Developing Excellence (TIDE) Program is an opportunity to be involved in the continuous improvement of the MD curriculum in the School of Medicine. This is a great chance to observe other teachers and to expand the educational activities on your CV. You may also pick up skills to improve your own teaching in the process. The SOM Educational Development Subcommittee has begun a peer teaching observation program for pre-clinical lectures for MD students in the first 18 months of medical school, and they are looking for passionate educators from all parts of the health sciences center to join them as lecture reviewers. DOM Honors & Recognitions Jason Schneider, MD, (General Internal Medicine), was selected to receive the Allen Thornell Political Advancement Award from Georgia Equality. Dr. Schneider was honored for his leadership in opening and operating the Grady Gender Clinic and for playing a pivotal role in organizing and mobilizing healthcare professionals to advocate against
restrictions on gender-affirming care during past legislative sessions. Sara Auld, MD, MSc, (Pulmonary, Allergy, Critical Care, and Sleep Medicine) received the 2024 Emory University Hospital Midtown Palliative Care Champion Award. This award, presented by the EUHM Palliative Care Team, recognizes the outstanding dedication and compassionate care provided to patients living with serious illness and their families. Dr. Auld exemplifies commitment and excellence in her profession. Each year, learners nominate and vote in the Foundations Faculty Awards in four categories.
