New Plan Year, New Medical ID Cards

If you're enrolled in a University-sponsored medical plan with UPMC Health Plan, you and your dependents should have recently received new Medical ID cards, which look similar to this:

Please destroy any old Medical ID cards and start using the new ones, which list our new medical plan names (name change only, no change to plan designs) and, on the reverse side, the information for our new prescription drug provider, CVS Caremark.

• To ensure proper billing/payment:
  • Anytime you're filling a new prescription, please give the pharmacy your new card.
  • Contact any pharmacy for which you have existing prescriptions with automatic refills and provide them a copy of your new card.
• Check participating pharmacies on CVS Caremark's Understanding your Rx Plan page.

If you did not receive your new Medical ID card, you can request another hard copy from UPMC Member Services at 888-499-6885. You can also download a digital version through the UPMC Health Plan mobile app, or follow these instructions to download a temporary medical card.
