STORY AND CASE

A 40-year-old female with a history of Wolff Parkinson White syndrome presented with a subacute progressive muscle weakness in the proximal upper and lower extremities bilaterally that was associated with ulcerative skin lesions on the chest and thighs, as well as a solid food dysphagia. Her vital signs upon admission to the hospital were within normal limits, and a focused physical examination revealed patchy hypopigmented areas in various stages of scarring that were located on the anterior and posterior aspects of the upper trunk, along with erythematous active ulcerative lesions in the axillary areas bilaterally. She was noted to have bilateral hand synovitis in multiple joints and a proximal 3/5 upper and lower extremity muscle weakness bilaterally.

Laboratory findings were notable for creatine kinase (CK) 4848 U/L, AST 200 IU/L, ALT 75 IU/L, ESR 45 mm/hr, CRP 73 mg/L, +ANA 1:1280 Speckled, high positive anti-NXP-2 (nuclear matrix protein 2) antibody, and an elevation of the following: aldolase, anti-RNP, anti-Smith, and anti-SSA IgG antibodies.

Muscle biopsy was performed, and the results are provided below:

Abnormality (left, H&E) highlighted by LCA immunohistochemistry (middle), with fascicle-to-fascicle size variability (right, ATPase).

Pathology report was consistent with a presence of degenerating and atrophic fibers, myo-phagocytosis, myocyte necrosis and no regeneration or primary inflammation. 

The regional increase in connective tissue suggested process had been ongoing for months to years.

Research 

2022 DOM FAME Grant Applications 

Deadlines: Tuesday, January 18 (LOI) ǀ Monday, February 28 (Full application) 

The DOM FAME grant is designed to provide support for clinical faculty to dedicate up to 20 percent of their professional time to scholarly activity including research, education, mentoring, and quality improvement projects.

One FAME grant will be awarded by the Department of Medicine, with additional awards supported by the following divisions: Digestive Diseases (1), General Internal Medicine (1), Geriatrics (1), Hospital Medicine (2), and Renal Medicine (1).

Candidates must submit an email to DOMResearch@emory.edu stating the name of the candidate, the candidate’s division, and the proposed title. This email serves as a “letter” of intent and is a required part of the application process.

 

2022 FAME Grant Writing Workshop Series 

Friday, January 14, 21, 28, 2022 ǀ 8 - 9 a.m. ǀ Zoom

This series provides the opportunity to learn essential grant writing skills from subject matter experts. While this series is optional, it is highly encouraged. Should you choose not to attend, you are still permitted to submit a FAME grant application.

Wellness and Community 

Physician Quick Consult
PQCs are 20-minute individual, confidential consultations with a FSAP licensed mental health professional to discuss current concerns (e.g., burnout) and to identify well-being strategies. Virtual consults offered M-F 7 a.m. - 6 p.m.

Physician Pandemic Support Group (Access via Zoom)
This weekly drop-in group is a place for support and open discussion with colleagues. Led by an FSAP psychologist, the group meets Thursdays from 5:15 - 6:15 p.m. The group structure is mindfulness instruction and practice and open discussion. Not interested in the mindfulness practice? Just log in at 5:30 p.m.

Weekly Refresh (Access via Zoom)
Join FSAP for a 15-minute virtual meditation and resilience tip for the day, held every Wednesday from 4:30-4:45 p.m. Questions?
