Letter from the Division Director

Dear Colleagues,

I am excited to share this year's final newsletter with you. Congratulations to all our faculty, fellows, and staff for their accomplishments this year! As we begin the holiday season, I want to thank each one of you for your many contributions to our division. This year, we have expanded our clinical, education and research enterprises with the addition of outstanding faculty, fellows and staff. We have launched our GI hospitalist program and the Intestinal Ultrasound program as part of the IBD center. We had several fun divisional events this year including the CCFA walk, our fellowship graduation and the faculty and fellows pickleball get together. We are happy to share this newsletter with our alumni and I look forward to engaging with each of you to share new initiatives to support our various educational and research activities in the division.

Research Highlights

The IMPACT study aims to enroll up to 30 patients, comprising 15 patients with inoperable locally advanced disease, and 15 patients with metastatic disease, across multiple centers in the U.S., Canada, and Israel.

Eligible patients must have newly diagnosed, histologically confirmed pancreatic adenocarcinoma and must be inoperable, non-irradiated, and either chemotherapy-naïve or within the first four cycles of their initial chemotherapy regimen. Patients who have undergone prior surgery or received radiation are excluded.

Patients will continue receiving their standard-of-care chemotherapy throughout the study (mFOLFIRINOX), and Alpha DaRT sources will be implanted into the primary tumor using ultrasound-guided endoscopy. Follow-up will continue up to six months after enrollment.

The primary objectives of the study are to assess the feasibility of Alpha DaRT source implantation and to evaluate its safety and tolerability, specifically monitoring for Grade 3 or higher adverse events related to the device. Secondary objectives include evaluating local tumor response using RECIST criteria and/or volumetric imaging, measuring time to local progression, monitoring progression-free survival and overall survival, and collecting patient-reported outcomes related to pain control. In addition, patients with inoperable locally advanced disease will be evaluated based on the percentage of patients who become surgically resectable after treatment with Alpha DaRT. ESD-EMR Hands-On Workshop

Need endoscopic therapies for large colon polyps, early-stage colorectal cancer, localized early stage esophageal or stomach cancer, or precancerous conditions of the gastrointestinal (GI) tract?

Rest assured, our interventional GI team has you covered! From endoscopic submucosal resection (EMR) to the endoscopic submucosal dissection (ESD) to hybrid procedures, our Endo Unit team at EUH is fully equipped with the expertise, technology and training necessary to perform these minimally invasive, innovative, organ preserving, novel interventions, to remove precancerous and early-stage cancerous growths in the GI tract, thereby sparing patients from undergoing major surgery. These procedures enable effective removal of precancerous and early cancerous lesions while avoiding the need for major surgery, thereby allowing for faster recovery, shorter hospital stays, and earlier return to daily activities. The success of this complex EMR and ESD program reflects the dedication of our highly trained staff and the institution’s commitment to innovation, patient-centered care, and excellence in therapeutic endoscopy.

In an effort to advance our understanding of complex endoscopic procedures and associated conditions, enhance technical proficiency, anticipate potential complications, and develop strategies for their effective management, the Endoscopy Unit at EUH, in collaboration with several industry partners, organized a comprehensive EMR–ESD hands-on skills workshop on Saturday, October 18, 2025. The event garnered strong participation from endoscopy technicians and nursing staff within the Endoscopy Unit, all of whom engaged in individualized, hands-on training with various ESD knives, snares, coagulation devices, and submucosal lifting agents. Using gel-based tissue models, participants were able to simulate real-life procedural scenarios, reinforcing both technical competence and confidence in managing advanced endoscopic interventions.

The workshop co-directed by Sailaja Pisipati, MD and Andrew Yu, MD, brought together dedicated endoscopy technicians and nurses in an inspiring display of teamwork, learning and shared commitment to excellence. Through hands-on sessions and collaborative demonstrations, participants strengthened not only their technical skills but also their collective confidence in supporting advanced endoscopic procedures. The atmosphere was one of enthusiasm and unity, as team members encouraged one another, exchanged ideas, and celebrated our first anniversary following the launch of our ESD program together. The event underscored the vital role of continuous education and teamwork in delivering the highest standards of patient care, leaving participants motivated and proud to be part of a forward-thinking, compassionate healthcare team.

DR. Rhim's Research

Andrew D. Rhim, MD - Dr. Rhim's research interests are in the development of novel therapeutics and diagnostic tools for pancreas, colon and other GI cancers. In collaboration with multiple groups, he established that endogenous retroelements (EREs; also encompasses retrotransposons, satellite/repeat RNAs and endogenous retroviruses) in the initiation and progression of various cancers. These EREs resemble primitive viral RNA sequences and mimic viral infection while also leading to chromosome instability. Dr. Rhim's work has led to the discovery of a class of molecules called Binders of Endogenous Retroelements (B-EREs) which are pivotal in protecting the host cell from the immune system and chromosome instability from these EREs and represent novel drug targets for cancer. One example is APOBEC3A, a B-ERE required for genomic progression into late stage disease and development of multidrug resistance in pancreas, lung and ovarian cancers (Wormann, et al. Nature Cancer 2021). In addition to developing novel inhibitors of APOBEC3A, Dr. Rhim seeks to repurpose FDA-approved drugs used in other diseases, such as HIV, that also target EREs and B-EREs for the treatment of cancer in a series of novel clinical trials

Recap of the Third Annual Julius Wenger Lecture

The Third Annual Julius Wenger Memorial Lecture in GI Endoscopy, held on Monday, December 8, was a great success! The day began with visiting guest speaker Carol A. Burke, MD, from the Cleveland Clinic, who delivered an informal session on “Career Considerations for GI Fellows” to our trainees.

The fellows found her insights especially valuable. For the main lecture, Dr. Burke presented “When to Suspect a Hereditary Adenomatous Polyposis Syndrome: A Case-Based Approach to Diagnosis and Management.” Her talk was exceptionally well-received by faculty, fellows, and all attendees. The event concluded with a dinner celebrating the legacy of Professor Emeritus Nanette Wenger, MD (Cardiology), at South City Kitchen in Midtown, providing a warm and celebratory end to the evening.
