"When it's not pancreatitis, don't brush it off"

STORY AND CASE:
The patient was a 72-year-old male with medical history of hypertension. He presented with fever and dull epigastric pain for 3 days. Physical exam was significant for epigastric and left periumbilical tenderness. He was febrile with a temperature of 38.4° F. Laboratory data revealed elevated white blood cells (18.0/μL), elevated inflammatory markers (ESR 39 mm/hr, CRP 134.80 mg/L), normal lipase of 27 unit/L, and a triglyceride level of 94 mg/dl. A CT scan of his abdomen and pelvis with intravenous (IV) contrast showed peripancreatic fat stranding around the head and uncinate process of the pancreas, suggesting acute pancreatitis, and inflammatory stranding adjacent to the third part of the duodenum, which was thought to be related to the pancreatic process. Gallbladder appeared normal on abdominal ultrasound and hepatobiliary scan. He was started on conservative management with bowel rest, IV fluid and pain medications.

The patient’s normal lipase level, lack of risk factors for pancreatitis including alcohol consumption, and absence of gallstones prompted the evaluation for another etiology of his pain. Esophagogastroduodenoscopy was pursued next and showed erythematous mucosa in the gastric antrum.  MRI of the abdomen with was then performed and showed a perforating duodenal foreign body (possibly a fishbone) with transmural extension into adjacent mesentery, causing marked focal duodenitis and a peri-duodenal phlegmon. There was no evidence of pancreatitis on MRI.

There is so much to celebrate! We received 108 nominations for the faculty and staff awards, 25 recognitions for clinical distinctions, and 32 promotions.

We will honor all of these incredible colleagues and their accomplishments on August 30th at the Honors and Awards Celebration. Details forthcoming.

Let's Talk Promotions

Dr. Michael Saenger, VA Primary Care and Quality, Department of Medicine, was approved for promotion to Associate Professor, effective September 1, 2022. Congratulations, Michael!

We asked Dr. Saenger to share his advice to those currently or soon to be on the promotion journey:

"Get engaged in both your passions and in open doors to serve, as new passions and skills emerge. Document what you're doing - all the little conferences, including feedback evaluations from participants. Publish what you're interested in and pursuing — join a peer writing group, get feedback, and collaborate." Dr. Saenger added some extra advice for the DOM saying, "Keep investing in faculty, including through direct coaching and editing like Alia Kamel's much needed help." Admittedly we're biased, Dr. Saenger, but we are very thankful for Alia! Thanks for your advice and feedback!

New: DOM Research Ride Podcast!

The Department of Medicine's Strategic Advisory Research Team (StART) is excited to announce the launch of its new DOM Research Ride podcast hosted by Dr. Charlie Searles. The podcast highlights our department's brilliant and passionate researchers and their cross-cutting research programs. 2022 Research Day Call for Abstracts

Deadline: Monday, August 22 at 11:59 p.m. EST.
Late submissions will not be accepted.

The Department of Medicine is now accepting abstracts for the 15th annual Research Day, to be held Thursday, October 27 from 8 a.m. to 5 p.m. Currently, we are planning to hold the event in-person at the Emory Student Center (subject to Emory Pandemic Operating Status). All Department of Medicine faculty, fellows, residents, and students (whose primary mentor is a member of the Department of Medicine) are invited to submit an abstract. There is a limit of two abstracts per presenting author.

 

2022 Discovery Phase Research Mentors and Projects 

We are looking for research active faculty to participate in the Discovery Phase of the Medical School Curriculum by mentoring a medical student research project. Topics in clinical, translational, basic science, or in the medical humanities, including creative writing, medical history, business, or law, as they pertain to medicine, are all potential areas as long as they lead to a scholarly product. Discovery is a required five-month phase of the curriculum during which students do full time research under the direction of their chosen mentor. Students have no responsibilities during this time other than research, so projects should be substantial enough to merit 5 months of full-time effort, but also should be feasible for what is not a long period in research terms. If you have a project in mind and would like to act as a mentor, please submit your research project.

The Faculty Staff Assistance Program (FSAP) is offering individual well-being check-ins for staff and faculty to gauge emotional health/coping and identify strategies for enhancing resilience, especially pertaining to transition. Mindfulness and a Weekly Refresh. Every Wednesday at 1:00 pm you can learn the basics of mindfulness. Then at 1:15, you can take part in a guided practice. Later in the day, at 4:30 pm, you can reconnect and join in a virtual meditation and resilience tip of the day. These three 15-minute sessions are offered every Wednesday at 1:00 pm, 1:15 pm, and 4:30 pm. Click on the time to go directly to the Zoom session.
