We are writing to inform you of an important update to our IACUC compliance reporting procedures. Effective immediately, we are implementing a new self-report form for self-reporting any IACUC compliance issues.
Purpose of the New Form:
This standardized, secure form has been designed to streamline our compliance reporting process and ensure that all necessary information is consistently captured when issues arise. The new system will help us maintain our commitment to animal welfare and regulatory compliance while making the reporting process more straightforward for researchers.

• Improved documentation and tracking of compliance matters
• Consistent handling of reports across all research groups
Implementation: The new form is now available on the IACUC homepage. Click on the Self-Report Link in the drop-down menu, under the Compliance tab. Please use this form going forward for any self-reporting of IACUC compliance issues, including protocol deviations, adverse events, or other concerns.
