Annual Declaration

You are receiving this email because you have not yet submitted an annual declaration of financial interest in a pharmacy business for the period 1 May 2017 to 30 April 2018.

In May this year you were sent an Annual Declaration form to complete and return to the Pharmacy Council by 30 June 2018. This is legal requirement for all persons who have a financial interest in a NSW pharmacy business.

The majority of pharmacists with a financial interest submitted the required information by the due date.  Our records indicate that you have not yet submitted your completed annual declaration, despite an overdue notice being sent to you in earlier this month.

Under the Health Practitioner Regulation National Law (NSW) a maximum penalty of $2,200.00 applies for failure to submit a declaration by the due date.

You are advised to return your completed annual declaration without further delay. It should be noted that continued failure to provide a completed annual declaration may result in the Council taking further action, which may include prosecution before the Local Court, referral to the Health Care Complaints Commission or dealing with the matter as a complaint of unsatisfactory professional conduct.
