Emory Division of Digestive Diseases Monthly Clinical Vignette Conference "Not all colitis is the same" Submitted by: STORY AND CASE: A 24-year-old female presented to the emergency room with complaints of worsening rectal bleeding. She has no known past medical history, and reports symptoms of abdominal pain, cramping, and urgency with defecation for the past several weeks. She has associated poor oral intake and unintentional weight loss of approximately 10 pounds. She presented to an outside hospital a few weeks prior to presentation and was prescribed a course of Levaquin and Clindamycin. She reported her symptoms worsened after the antibiotics. She denied any outside travel, or sick contacts. She denied any alcohol use, smoking or any illicit drug use. Review of symptoms indicated bilateral knee pain and wrist pain. On presentation to our facility, she was hemodynamically stable and afebrile. CBC noted a hemoglobin of 9 g/dL and platelet count of 400 K/mcL. Her chemistry was notable for albumin of 2.5 g/dL, ESR of 40 mm/h, and CRP of 50 mg/L. She had diffuse abdominal tenderness, no guarding or rigidity on exam. She completed a CT abdomen in the emergency room which noted diffuse bowel wall thickening. She was admitted and underwent an
inpatient colonoscopy, which noted severe colitis. Following the results of abdominal computed tomography patient had serum protein electrophoresis performed, which revealed elevated free kappa light chains at 36 mg/L, prompting a bone marrow biopsy. The results of the bone marrow biopsy revealed B-cell acute lymphoblastic leukemia. SOM Leader Update: Faculty Leadership Transitions Kathy Griendling, PhD, has been appointed the Executive Associate Dean of Faculty Academic Affairs and Professional Development within the School of Medicine. Kathy is a nationally recognized scientist and is extraordinarily qualified for this leadership role. Since 2014, Kathy has served as Associate Dean for Faculty Affairs and Professional Development and has spent her career mentoring others and advocating for the development and recognition of our faculty.
