News updates and your September 2021 monthly dataCoordinate My Care rolls out GP Digest FunctionCMC has reinstated the GP Digest function of the service, following some new system enhancements. This means that GP Practices who have an ISA and who have opted to receive communications from CMC, will receive email alerts highlighting changes in their registered patient’s CMC care plan, including care plan updates, recently deceased notifications, or withdrawal of consent by the patient. For GP practices with nhs.net email addresses, GP Digest will detail the patient’s personal details, for those without an nhs.net email address the patient’s CMC record number will be hyperlinked to the CMC dashboard, where they will need to log in securely before reviewing the information. The reinstation of GP Digest follows feedback from GPs to CMC who have stated that it would benefit them and help to manage the CMC service within their practice. CMC Lunch & Learn: Collaboration with PaediatricsThe October Lunch & Learn session focused on the benefits of CMC for paediatric patients. We were joined by The North Thames Paediatric Network and reported on key data findings of paediatric patients with a CMC care plan. The next Lunch & Learn is currently scheduled for Wednesday 10th November and will be hosted by Dr Kirsty Boyd, Palliative Reader from the Usher Institute at Edinburgh University. Completing MyCMC plans – CALL TO ACTIONThere are currently a significant number of incomplete MyCMC plans on the system, which means patients’ wishes and preferences are not being met when they call upon the urgent care services. This could result in patients not achieving their preferred place of care or death, unnecessary hospital admissions and ultimately increased costs to the NHS. It could also result in patient and family complaints. Once a patient has completed their MyCMC questionnaire, this information is then transferred digitally to the patient’s registered GP surgery to finalise and approve. There are then just three key pieces of information to add:
Once this information is added, approve the plan by clicking on the approve button, this will then immediately go live and be available to view by all Urgent Care Providers.
