Dr Boyd focused her session on Advance Care Planning and the REDMAP Framework that she has developed to help conversations around this. The next Lunch & Learn is scheduled for Wednesday 15th December and will be hosted by Professor Paddy Stone, Marie Curie Chair in Palliative and End of Life Care. Coordinate My Care Partner Engagement WorkMarie Curie LondonWe are pleased to announce the success of the Marie Curie Virtual Advance Care Planning service. Between April 2020 - March 2021 during the height of the pandemic the Marie Curie ACP Service created 1,361 new care plans and reviewed and updated 313 existing plans across 11 London Boroughs. This is an exemplary example of how digital access to NHS urgent care services, via Coordinate My Care, helps patients and their carers be central, to their own care. North Middlesex University Hospital NHS TrustNMUH ran a Quality Improvement Project to increase staff access and the number of patient CMC urgent care plans created in the hospital. CMC logins were created for 207 MDT members of staff, enabling access to existing care plans as well as an existing tool to document and communicate new plans made with patients and their carers / loved ones. Completing MyCMC plans - CALL TO ACTIONThere are currently a significant number of incomplete MyCMC plans on the system, which means patients’ wishes and preferences are not being met when they call upon the urgent care services. This could result in patients not achieving their preferred place of care or death, unnecessary hospital admissions and ultimately increased costs to the NHS. It could also result in patient and family complaints. Once a patient has completed their MyCMC questionnaire, this information is then transferred digitally to the patient’s registered GP surgery to finalise and approve. There are then just three key pieces of information to add:
Once this information is added, approve the plan by clicking on the approve button, this will then immediately go live and be available to view by all Urgent Care Providers. CMC Training & EducationCMC System Training takes place virtually twice weekly on Tuesdays at 09.30 - click here to register, and Thursdays at 13.00 - click here to register. During the training you can learn about the CMC service, how it works and how you can create, view and update a care plan. This training is mandatory to get access to the service. You can attend a weekly live webinar or arrange a bespoke session for 4 or more attendees.
