The Manage My Care App and web portal

  • Helps patients and their families and carers view their public outpatient appointments and referrals;
  • Displays appointments for PUBLIC outpatient services, (so no privately delivered appointments will appear); and
  • Draws on information contained in WebPAS (which is a tool for managing admissions and discharges). While WACHS uses a number of other systems as well, only WebPAS data, appointments and referrals will appear in the MMC App.

Visit the website here.

The MWGHPN is active on Facebook, members can keep up to date with local educational and social events being held in their region.

My Community Directory

My Community Directory is an online directory of hundreds of local health, social and community services across the State that WA Primary Health Alliance (WAPHA) has partnered with and invested in, to address inequity and access. These services provide high-quality treatment and support across the lifespan for both physical and mental health. Find out how they keep Western Australians well and out of the hospital and improve access to primary healthcare.

Features and functions within the directory have been developed to connect people to and between services, supporting three core users:

  • Community members
  • Service providers
  • Service planners

My Community Directory is free for service providers to:

  • List your service and keep the listing up to date.
  • Connect to other services for the benefit of your own professional networks and to support connections for service users.
  • See what other services are within a search area.

Community-based groups, organisations and individuals offering free or low-cost services or activities to Midwest-Gascoyne residents can register on the directory and promote their services free of charge.

List your information via this link.
