Your Annual Wellness Visit can help you and your doctor create a personal wellness plan based on your current health and risk factors. This can help to catch health problems early when they are easier to manage.

1. Transfer your medical records
   If you haven’t transferred your medical records, complete our Transfer of Medical Records form located on our website.
    
2. Schedule your annual wellness visit
   The Annual Wellness Visit is the perfect way to help us establish a baseline of care and may be covered under your health plan. Once you check with your health plan to determine benefits, call your Primary Care Physician’s office to schedule your appointment today.
    
3. Count on us to help you navigate the healthcare system
   Our team of doctors, advanced practice clinicians, medical assistants and front office staff will ensure you receive the best care during your office visit. Our team will order needed tests such as blood draws, cholesterol screenings and/or specialist’s visits. We will work together to ensure you experience seamless care.
