In 2025, CQC initiatives drove measurable improvements in quality, equity and outcomes across California through aligned improvement efforts involving health plans and delivery system partners serving more than 20 million Californians.
Today, we’re proud to share CQC’s 2025 Impact Report, highlighting how collective action translated into measurable progress across California’s health care system. We’re grateful to the health plans, providers, purchasers and partners whose collaboration continues to make this work possible.
2025 Highlights
- 20M+ Californians impacted through aligned health plan and delivery system partnerships
- 900,000+ patients directly reached through CQC initiatives
- 25.1% relative improvement in depression screening and follow-up, resulting in more than 64,000 additional patients receiving care
- Three commercial health plans co-implemented a shared value-based payment model demonstration
- Five Medi-Cal health plans aligned around quality reporting and shared data infrastructure to reduce administrative burden
- 31 independent practices achieved measurable improvements in blood pressure control, cancer screening and diabetes care
