East Riding Family Nurse Partnership DecommissioningThis announcement has been shared by Humber Teaching NHS Foundation Trust on behalf of Andy Kingdom, Director of Public Health at East Riding of Yorkshire Council. It outlines the council's decision to decommission the Family Nurse Partnership (FNP) element of the Integrated Specialist Public Health Nursing Service in the East Riding. The letter explains the reasons for the change, the support arrangements that will remain in place through the Healthy Child Programme, and the steps being taken by the council, the Trust and partner organisations to ensure families experience a smooth transition with no interruption to care and support. "Dear Colleagues, I am writing to inform you that a decision has been made to decommission the Family Nurse Partnership (FNP) element of our Integrated Specialist Public Health Nursing Service (ISPHNS) in the East Riding of Yorkshire. Please know this decision has not been taken lightly and follows careful review of financial pressures and funding affecting both the local authority and current provider, Humber Teaching NHS Foundation Trust, whilst ensuring we continue to meet the needs of our families. As the FNP programme comes to an end, we remain committed to ensuring that all children, young people and families continue to receive appropriate support through the national Healthy Child Programme (HCP) and wider early help services. For clarity, any future referrals that would be directed to the FNP will be supported by the Healthy Child Programme. Each family currently supported by the FNP will receive an individual transition plan, informed by an assessment of their needs, strengths and any identified risks. Our Family Nurse Practitioners will work closely with Health Visiting services and other relevant health and local authority professionals to ensure a coordinated and seamless transfer of care. Wherever possible, families will be offered a joint handover of care with their existing Family Nurse to support continuity of relationships and ongoing engagement. Through our Healthy Child Programme offer, families will continue to have access to universal, targeted and specialist health visiting services, including health and development reviews, parenting support, advice on infant feeding, emotional wellbeing support, safeguarding oversight, and access to wider community and specialist services where required. Families with additional needs will be supported to access targeted services through agreed referral pathways and multiagency arrangements. We want to make it clear that we recognise the importance of maintaining trusted relationships during this transition. Families will be supported to understand the services available to them, how to access support when needed, and how to continue building on the progress and achievements made during their time with the FNP. Our shared priority is to ensure that no family experiences a gap in support and that babies and young children continue to receive safe, effective and responsive care through the Healthy Child Programme and partner services. We recognise the significant contribution that the FNP team have made to improving outcomes for young parents and their children across the East Riding and we would like to thank colleagues and partners for their commitment and support over many years." Each month we will be sharing patient experiences from our different services, alongside a patient comment. 91.7% of respondents (326 total) thought they were involved in their care and listened to within the Community and Primary Care Division in August 2026.
