Inside Look: Exploring the New Payment Model Demonstration Project

Launching next month, the California Advanced Primary Initiative's Payment Model Demonstration Project is set to significantly enhance primary care in California. This innovative project will test a unified value-based payment model by three health plans — Aetna, Blue Shield of California and Health Net — across up to 30 independent practices. Equipping providers with essential resources and aligned payment structures across payers, the project aims to elevate the quality of patient care and achieve superior health outcomes.

Key Features:

  • Innovative funding models. Focusing on prospective and value-based payments to align health care incentives with patient health outcomes.
  • Technical Support for independent practices. Providing tailored technical assistance to help small and medium-sized practices thrive under new payment structures.
  • Common reporting platform. Enabling small practices to report and view data quickly to close care gaps for patients across multiple health plans in the payment demo project.

Strengthening Health Equity through Primary Care Transformation

Primary care serves as the cornerstone of health care delivery, especially when addressing long-standing disparities in access and quality. To fully realize its potential in advancing health equity, the California Health Care Foundation, in collaboration with Mathematica, highlights the importance of investing in primary care through a new report. The report cites the California Advanced Primary Care Initiative as a prime example of how collaboration among health plans, providers and purchasers can successfully address health inequities.
