The pilot will:
CQC and the Integrated Healthcare Association (IHA) have partnered to determine a process for measuring performance at individual provider locations across the state of California. Additionally, recommendations for incorporating race, ethnicity and language data into the APC pilot to better identify and reduce inequities in care and health outcomes will be developed. Creating a Roadmap for Health Plans and ProvidersIn partnership with IHA, CQC is also bringing together health plans and delegated provider groups, representing most primary care payers in California, to develop aligned goals and action steps through 2025 to accelerate system-wide implementation of Advanced Primary Care standards. Through CQC and IHA’s coordination, payers will collectively commit to grow value-based primary care payment and investment in California.
