Advancing Whole-Person, Integrated Primary Care Across California
CQC has updated its Advanced Primary Care framework to better support whole-person, integrated care. The framework brings together a shared set of attributes defining high-quality primary care, a standardized measure set for evaluating performance and new operational examples designed to help organizations put the attributes into practice.
The new operational guidance offers practical approaches for integrating physical health, behavioral health and social needs that organizations can adapt based on their patients, care teams and available resources.
CalOptima Health Hosts CQC Improvement Coaching Workshop
Participants use creative, hands-on exercises to explore quality improvement concepts during CQC's Improvement Coaching Workshop.
CQC recently brought its Improvement Coaching Workshop to teams from six primary care clinics at CalOptima Health, building practical skills to identify root causes of care gaps, test small changes and strengthen chronic disease care. The hands-on workshop equips care teams with tools they can apply to improvement efforts within their own organizations. CQC's next Improvement Coaching Workshop takes place Oct. 15 in Sacramento.

DHCS Expands Behavioral Health Data in Medi-Cal Connect
The California Department of Health Care Services (DHCS) has added new behavioral health features and data to Medi-Cal Connect to support quality improvement, population health and care management. Updates include Behavioral Health Transformation and BH-CONNECT Incentive Program measures and rates, new behavioral health data files and additional Risk Stratification, Segmentation, and Tiering data.
