Reminder to upload your COVID-19 Vaccination Information on Calvary Supplier Central

Dear Valued Supplier,

Thank you for your continued commitment to keeping our staff, residents and clients safe. 
This correspondence is a reminder to suppliers and contractors that certification of COVID-19 vaccination for yourself, your staff and sub-contractors is now due.

Calvary’s Mandatory COVID-19 Vaccination Policy
In accordance with the Calvary COVID-19 Mandatory Vaccination Policy, which aims to correlate with State and Territory COVID-19 mandatory vaccination Health Directives:

From 1 November 2021 all suppliers and contractors visiting or working at Calvary facilities, on Calvary premises or providing work to Calvary clients are required to have received dose #1 of an approved COVID-19 vaccine. 

For suppliers and contractors visiting or working at Calvary Aged Care facilities, there is a further requirement that dose #2 of an approved COVID-19 vaccine has been received.

In order to comply with State and Territory Health Directives, Calvary must collect, record and hold the vaccination information of all workers, including suppliers and contractors to Calvary.

How do I provide certification of vaccination to Calvary?
Calvary now collects all supplier and contractor information, including insurance certificates, vaccination certificates and statutory declarations through Calvary Supplier Central.

What do I need to supply to evidence COVID-19 Vaccination Status?
Calvary are required under Public Health Orders to collect, record and hold vaccination information for all workers who enter or work, in a Calvary facility, on Calvary premises, or provide care to a Calvary client.

Calvary request that, as a supplier to Calvary, you provide either vaccination certificates or a statutory declaration as evidence that all your staff and any subcontractors (engaged by yourself) who may enter or work in a Calvary facility, on Calvary premises or provide care to a Calvary client, are vaccinated. 

Suppliers visiting or working in Calvary Aged Care facilities: For suppliers visiting or working in Calvary Aged Care facilities, please evidence via submission of vaccination certificates or this statutory declaration  to Calvary Supplier Central that all your staff and subcontractors who enter Calvary Aged Care premises have received dose #2 of an approved COVID-19 vaccination by 1 November 2021.

Suppliers visiting or working in all other Calvary facilities or with Calvary clients
For suppliers visiting or working in, other than Calvary Aged Care facilities, please provide evidence via submission of vaccination certificates or this statutory declaration to Calvary Supplier Central that all your staff and subcontractors who enter either a Calvary facilities premises or work with a Calvary client have received dose #1 of an approved COVID-19 vaccination by 1 November 2021.

What do I do if I can’t access Calvary Supplier Central or am having trouble uploading my documents?

If you have not received your registration email, or you have inadvertently deleted earlier emails, or are having trouble registering, please email the National Procurement Team at nationalprocurement@calvarycare.org.au with details of your enquiry and one of our team members will be in touch.

Site level access
Calvary’s hospitals, aged care facilities and corporate offices may seek to verify vaccination status to allow you access to the facility. Please ensure that you and your workers have on their person one of the following:

•    online immunisation history statement
•    COVID-19 digital certificate via the State or Territory’s Service app
•    COVID-19 digital certificate via the Australian Immunisation Register
OR, if applicable:
•    medical contraindication certificate
•    medical clearance form.

Note that evidence of medical exemptions must be from a medical practitioner in the form approved by the applicable State’s or Territory’s Chief Health Officer.
