New Participant Information and Consent Form templates

We have adopted a new Particant Information and Consent Form (PICF) template. This PICF has been carefully developed by CT:IQ with consumer input. It can be used for both general research and clinical trials. More information here.

Research Office staff have developed a PICF template for anonymous surveys, tailored for use at AWH.

The Research Office PD and Education page has links to the PICF templates and guidance on PICFs, such as plain writing and inclusion of minority groups.
