The California Medicare Collaborative (Collaborative)'s new issue brief outlines a strategic pathway for adapting Medicare services to the complex needs of California's rapidly aging population, expected to reach a quarter of the
state's demographic by 2030. California's growing Medicare population presents an opportunity for innovative system shifts that will allow individuals to age with dignity and receive the tailored health and long-term care they deserve. The issue brief provides actionable steps that California health care stakeholders can take to navigate policy, payment and care delivery challenges within the state’s Medicare system to: - Strengthen Chronic Illness Management and Care Coordination
- Enhance Access and Equity
- Improve Beneficiary Decision-Making
- Improve Screening and Supports for Cognitive and Behavioral Health
Spearheaded by CQC, The SCAN Foundation, Archstone Foundation, Gary and Mary West Foundation, and in partnership with the Department of Health Care Services Office of Medicare Innovation and Integration, the Collaborative launched in March 2024. This year-long, multi-stakeholder effort was dedicated to improving care for residents insured solely by Medicare, including duals, non-duals and those in both traditional Medicare and Medicare Advantage programs. 2025 CQC Network Convening
On March 4th, health care leaders and other stakeholders gathered in Burbank, CA, for the California Quality Collaborative’s 2025 Network Convening. Throughout the day, attendees explored innovative strategies for health care transformation, including integrating behavioral health into primary care, utilizing innovative payment models and enhancing patient engagement. Our keynote speaker Dr. Mark Ghaly, former Secretary of California Health and Human Services, encouraged the providers, payers and other partners present to seize the current positive momentum within California's health care system to drive advancements now and ensure lasting benefits, despite future uncertainties. Dr. Ghaly highlighted the following opportunities for improvement: - Empowering Providers: Elevate providers and care teams to operate at the top of their licensure, utilizing artificial intelligence to uptrain or cross-train care teams.
- Strengthening Community Health Workers: Recognize and enhance the value of community health workers in improving health care delivery and patient engagement.
- Patient-Centric Approach: Prioritize patients at the core of all health care strategies, ensuring their needs and perspectives help shape innovative solutions.
The convening showcased innovative solutions across California from organizations such as HealthNet, Vivant Health, Futuro Health and more, reinforcing a shared commitment to creating sustainable progress within California's health care landscape.
