Building upon our multi-stakeholder alignment work to strengthen primary care, the California Quality Collaborative (CQC) in partnership with the Integrated Healthcare Association (IHA) developed a common hybrid payment model incorporating key elements to enhance health equity.

In Case You Missed It: Introducing Our Payment Model Demonstration Project
As the next phase of the California Advanced Primary Care Initiative, the payment model demonstration project will test a common value-based payment model with approximately 30 independent practices to strengthen primary care delivery. - This project is a collaboration of CQC, IHA and four participating health plans: Aetna, Anthem, Blue Shield of California and Health Net.
