Designing an Effective Colorectal Cancer Outreach Campaign

Colorectal cancer is the second leading cause of death from cancer in the United States. Black Americans have a 20% higher incidence of colorectal cancer and a 40% higher death rate from the disease than white Americans, underlining the urgent need for action to reduce these inequities.

CQC has developed a guide to help health plans, provider organizations and individual providers design effective outreach campaigns to support colorectal cancer screening.

CQC is working to advance health equity through Equity and Quality at Independent Practices in LA County, a quality improvement collaborative for primary care practices and independent physician associations focused on reducing health disparities for Medi-Cal enrollees of color.

 
 
 

A Journey of Resilience and Advocacy in a Complex Health Care Landscape

When David Ford received a life-altering colorectal cancer diagnosis in 2015, he was thrust into the complex world of navigating the health care delivery system which revealed significant disparities especially prevalent in minority communities. His journey from patient to vocal advocate for health equity underscores the critical need for systemic change.

A Focus on Behavioral Health Integration 

The integration of behavioral health into primary care is crucial to ensuring the delivery of patient-centered, whole-person care that addresses both mental and physical health, improving patient outcomes. Through the Behavioral Health Integration Initiative, CQC is committed to accelerating integration efforts by small and independent primary care practices throughout California.

2024 Program Launches

CQC will introduce the following programs this year:

  • Behavioral Health Integration – Children and Youth Collaborative Learning Exchange. CQC will convene provider organizations experienced in integrating behavioral health for this demographic. Learn more.
  • Behavioral Health Integration Financing. CQC will conduct a state-wide landscape assessment project to identify, synthesize and communicate successful practices to ensure sustainable payment for behavioral health integration. Learn more.
