Strengthening Primary Care in CaliforniaAt the California Advanced Primary Care Initiative Summer Convening, health plan partners gathered to finalize preparations for the upcoming October 2024 launch of the Payment Model Demonstration Project, which will test a common value-based payment model in independent practices to strengthen primary care delivery. This data has been instrumental in driving accountability, public transparency and health equity.
