Updated LFP Payment Schedule

The purpose of this newsletter is to provide the latest updates on the implementation of the Longitudinal Family Physician (LFP) Payment Model. To submit questions or feedback on the payment model, please email LFP.Billing@doctorsofbc.ca.

 

Doctors of BC and BC Family Doctors continue to work with the Ministry of Health to improve, expand, and develop tools and supports for the LFP Payment Model. We continue to listen to your feedback, meeting regularly with the provincial government to address concerns. 

Today we are providing the following updates:

  • Updated LFP Payment Schedule 
  • EMR supports for patient panel submissions
  • New locum billing tools 
 

Updated LFP Payment Schedule 

The LFP Payment Schedule has been updated to improve guidance to physicians and provide additional information. The new payment schedule is effective August 9, 2023. Thank you to the physicians who have written to us with questions and suggestions. Changes to the payment schedule are guided by your feedback. Please remember to use the latest version of the LFP Payment Schedule to ensure you are billing accurately. 

There are three significant changes in this new payment schedule: 

  • A more comprehensive definition of “Empanelled Patient” has been included in the payment schedule to provide clarity about which patients are to be included on panel lists. The updated definition states: “Empanelled Patient” means an individual for whom a family physician has accepted responsibility to provide and coordinate longitudinal, relationship-based, comprehensive, family medicine care.

  • A new definition of “Longitudinal Family Physician Services” has been drafted to better reflect the type of care provided by longitudinal family physicians. The new definition states: “Longitudinal Family Physician Services” means the types of services typically provided by a family physician who provides longitudinal, relationship-based, comprehensive, family medicine care in a community based setting (including medically required services to beneficiaries), aligned with the attributes of a Patient Medical Home.

  • Clinical Administration Time Codes (98012/98042) have been updated to remove the following example of clinical administration activities: “Physician time required to confirm their patient panel as well as develop and submit an accurate list of Empanelled Patients for whom they have accepted responsibility.” Physicians will be compensated for the work involved in developing and submitting a list of empanelled patients through a new payment outside of the LFP Payment Model. The new payment will enable this panel management work to be done by physicians and/or delegated to clinic staff, as appropriate. The specifics of the new payment will be shared as soon as final details are confirmed.

BC Family Doctors’ web resources, including the Simplified LFP Guide and Billing Question Library, are being updated to reflect all of the changes to the payment schedule.

 

EMR supports for patient panel submissions

When enrolling in the LFP Payment Model, physicians commit to develop and submit an accurate list of empanelled patients for whom they have accepted responsibility. Submissions will be done as a staged process over the next few months. The volume and timing of panel submissions will be managed based on the capacity of MSP Teleplan, the system by which physicians submit billing claims.

All physicians are asked to attend one of the drop-in sessions before submitting their panel list. These sessions will be organized according to the EMR being used. This is to ensure that MSP Teleplan is not overwhelmed by a high volume of submissions. Support staff are also welcome to attend the drop-in sessions. We’ll be continuing to add additional EMR-specific drop-in sessions in the coming weeks.  

We will share information in the coming weeks about how physicians will be able to register to submit their panel on a specific date of their choosing. This process is currently being finalized in a continued effort to enable physicians to submit their panel while also maintaining the capacity of MSP Teleplan. Resources are available for physicians to review to support their panel submissions at a later date. 

MedAccess and Accuro users

To support physicians and their staff with submissions, we are hosting upcoming drop-in sessions for MedAccess and Accuro users who are ready or nearly ready to submit their panel information. 

Please attend a drop-in session when you are ready to submit your panel information and have completed panel clean-up. You will be asked to submit your panel in the one week following the session.

•    MedAccess drop-in session: Wednesday, August 16, 5:30pm – 6:30pm PT.
•    Accuro drop-in session: Thursday, August 17, 5:30pm –6:30pm PT. 

During these sessions, you will see a demonstration of the EMR workflow for panel submission. Staff will answer questions and provide guidance on next steps.

Additional sessions are being planned for these EMRs and others. Visit this webpage regularly for the latest schedule of EMR-specific webinars and drop-in sessions.

 

Other EMRs

More sessions will be held for physicians using an EMR other than MedAccess or Accuro. At this time, please do not submit your panel until you are able to attend a drop-in session for your EMR. 

The process for submitting panels has been validated with MedAccess, Profile, and Accuro EMRs, while work continues to progress with additional EMRs. Physicians that do not use an EMR with bulk claiming functionality will submit their panel via a third-party billing service. The service has been procured by the Ministry of Health and will be provided at no cost to physicians. More details on service and process will be available in the coming weeks.

We encourage all physicians to clean up their panels now, so you are ready to submit your panel list. To support you in preparing your accurate and up-to-date list of empanelled patients through your EMRs, self-serve guides and resources can be found on our Doctors Technology Office Health Technology Resources page. New LFP locum billing tools

BC Family Doctors continues to create and update billing resources to support family physicians billing under the LFP Payment Model. Since the July 1 changes to locum eligibility and billing, we have created and updated our locum billing tools to support LFP Locums and clinics. These billing tools include (member login required): 

•    Time and Patient Interaction Codes – Locum overview (updated)
•    Patient Interaction Codes Quick Reference Sheet – Locum (updated)
•    Sample Daysheet with Locum Time and Interaction Codes (new)

In addition, the Billing Question Library has been updated to reflect the locum changes. We encourage physicians who are enrolled in the LFP Payment Model to check the BC Family Doctors website on a regular basis. If you are not yet a member, you can join at any time.
