Finding Success in Practice Transformation

Physician practices today face significant challenges as they work to adapt to an ever-demanding health care environment and to improve the systems that enable the delivery of high-quality patient care.  

PBGH’s California Quality Collaborative (CQC) is dedicated to working with physician practices to develop and spread the most effective models aimed at achieving health care’s triple aim: reducing cost, improving population health and improving patient care quality and satisfaction. 

A fundamental way in which CQC supports provider organizations in their critical practice transformation work is through its multi-year technical assistance programs, the CalHIVE Improvement Collaboratives. 

Current CalHIVE Improvement Collaboratives:

  • CalHIVE Behavioral Health Integration Collaborative, a three-year program that is helping nine provider organizations pilot and spread behavioral health services within primary care. 
  • Equity and Quality at Independent Practices in LA County, a two-year quality improvement collaborative for small, independent, primary care practices and independent physician associations serving Medi-Cal enrollees of color in Los Angeles County.

These initiatives come on the heels of CQC's Practice Transformation Initiative, a four-year program that engaged 4,800 clinicians throughout California to improve measures of cost, quality and patient experience — resulting in $1.86 million in total cost savings. Golden Valley Health Center, a rural Federally Qualified Health Center participating in the initiative, improved patient screening and treatment for behavioral health by expanding virtual access.

Collaboration, Connection and Progress

Over the last month, CQC met with providers, health plans and partners to align stakeholders to a common goal and to collaborate, exchange ideas and advance their health care improvement work in support of health equity, behavioral health integration into primary care practices and primary care as a whole.

Patient-Centered, High-Quality Primary Care

"Through the California Advanced Primary Care Initiative, California Quality Collaborative and Integrated Healthcare Association are working together with a coalition of large commercial payers to strengthen the state's primary care delivery system, transforming it into a high-performing, value-based care model."

Crystal Eubanks, vice president of care transformation, spoke at the 2023 Civitas Networks for Health Annual Conference about how the innovative work of the California Advanced Primary Care Initiative can be used as a model for similar efforts across the United States.

Save the Date | Advanced Primary Care Measurement Results

Join CQC on December 7th for a webinar examining the results from CQC and partner Integrated Healthcare Association (IHA)’s measurement pilot. This pilot brought together four large health care purchasers in California to test a set of practice-level performance measures emphasizing patient experience and outcomes.   • A Birth Center Primer for Hospital Leaders (11/13): Free virtual learning series designed to help hospital leaders establish new birth centers and strengthen relationships with existing ones. Register and access recorded sessions of past webinars.
    • Apply | Action collaborative for birth centers and partner hospitals will launch in January 2024.
  • Second National Primary Care Transformation Summit (12/4-7): Registration is free for primary care practitioners. Multiple representatives from PBGH will present. Register for the summit.
