These initiatives come on the heels of CQC's Practice Transformation Initiative, a four-year program that engaged 4,800 clinicians throughout California to improve measures of cost, quality and patient experience — resulting in $1.86 million in total cost savings. Golden Valley Health Center, a rural Federally Qualified Health Center participating in the initiative, improved patient screening and treatment for behavioral health by expanding virtual access. This work can serve as a guide for provider organizations looking to make comprehensive, sustainable change.
