California Medicare Collaborative: Shaping the Future of Senior Care

California’s over-65 population is rapidly growing and diversifying, set to comprise a quarter of the state's population by 2030. Addressing their unique health and long-term care needs is becoming increasingly critical. To tackle this challenge, CQC, in partnership with The SCAN Foundation, Archstone Foundation, Gary and Mary West Foundation and the Department of Health Care Services Office of Medicare Innovation and Integration, has launched the California Medicare Collaborative.

This one-year stakeholder alignment effort aims to improve care for all Medicare enrollees, including duals, non-duals and those in traditional Medicare and Medicare Advantage programs. The collaborative will focus on four key areas:

  1. Cognitive and behavioral health
  2. Chronic illness management and care coordination
  3. Support of consumer choice in a confusing market
  4. Access, equity and disparities in care
