Stories of Transformation: Reddy Care Medical

The California Quality Collaborative’s Equity and Quality at Independent Practices in LA County (EQuIP-LA) program concluded in March with a final convening that celebrated two years of progress toward reducing disparities of care experienced by Medi-Cal enrollees of color.

Through the support of EQuIP-LA’s community-based coaching model, Reddy Care Medical strengthened care coordination and patient follow-up efforts, directly addressing gaps that contribute to disparities for Medi-Cal enrollees. Their journey reflects how focused quality improvement interventions—guided by real-time data insights and patient-centered strategies—can drive meaningful, sustainable change for historically underserved communities.

Approaches to Designing Equity-Centered Quality Improvement Projects

At the heart of health equity is intentional design. Equity-centered quality improvement (QI) projects prioritize the elimination of disparities by embedding equity into every stage of planning, implementation and evaluation.

This implementation spotlight outlines practical approaches to designing equity-centered QI efforts, drawing from lessons learned through EQuIP-LA. From co-designing interventions with patients to using disaggregated data to identify inequities, the strategies featured here are both actionable and adaptable.

As part of CQC’s broader commitment to advancing equitable care, this implementation spotlight supports health care organizations in centering communities that have been historically marginalized and underserved — and ensuring improvement work delivers real impact where it’s needed most.

Empowering Change: The Role of Community-Based Coaching in Health Care Transformation

Practice coaching is a proven driver of meaningful improvement in primary care, especially when rooted in the communities it serves. For years, CQC has supported health care organizations in adopting a community-based coaching model that fosters trust, accountability and sustainable change.

This implementation spotlight explores how community-based coaching supports the adoption of evidence-based care, grounded in lessons from EQuIP-LA.
