Celebrating the Triumph of the SFPN Strategy Session! 

We are thrilled to announce that the SFPN strategy session held on September 13 exceeded all expectations, paving the way for a brighter future for our organization. This dynamic event brought together SFPN Board members, harnessing their collective wisdom and innovative spirit to chart an inspiring course forward.

Our SFPN Strategy Session was a testament to collaboration, innovation, and a shared commitment to excellence. We cannot wait to embark on this exciting journey, fueled by the powerful vision crafted during this momentous event.

The SFPN is more determined to lead health care transformation in Scarborough along with our partners in order to make healthcare more accessible, equitable, and responsive to the diverse needs of our community. The AYA Circle of Care

Taibu is dedicated to providing timely and individualized intensive HIV case management and treatment for people of African descent who are newly diagnosed with HIV, at risk of HIV, or affected by HIV.

The Aya Project Team comprises of 
Nurse Practitioner, Registered 
Nurse, Social Worker and 
Community Navigator provides 
culturally appropriate Intensive 
Care Management services, wrap 
around support and systems 
navigation. Clients will also have access to the multidisciplinary team and 
programs at TAIBU (including 
Registered Dietitian, Chiropodist, 
Chiropractor and other health 
promotion programming).

'New' Virtual Urgent Care - CAMH

 

On August 28, CAMH launched Canada’s first and only same-day hospital-based Virtual Urgent Care Clinic, dedicated to providing urgent support for people in need of mental health, including addiction, advice. CAMH’s Virtual Urgent Care Clinic will be taking appointments from 9:00 a.m. to 5:00 p.m. through Virtual Urgent Care | CAMH

"Centralized Patient Scheduling" at SHN

SHN is transforming the way patients and families access services at Scarborough Health Network (SHN) by centralizing patient scheduling. Centralized Patient Scheduling (CPS) will go live in a phased approach, on September 27 and October 25. 

When SHN goes live with the new CPS model, patients will have a more convenient way to book, reschedule, or cancel their appointments through a single point of contact. This will be facilitated through the centralized office with dedicated scheduling agents and extended service hours, including evenings and weekends. A visual overview is attached to this email, to outline what this service transformation means for you and your patients. If you have any questions, please contact ptscheduling@shn.ca.

Secure Messaging Pilot with Pediatric & Palliative specialists

The SOHT Secure Messaging Pilot will support primary care to securely communicate with paediatric and palliative specialists for timely advice and care planning.  Leveraging Microsoft Teams, participants will be able to direct message their colleagues and share PHI in a manner that is PHIPA compliant.  We are continuing to seek out primary care physicians to participate in the pilot.  If interested please email Chris Alappat, Digital Health Lead at chris.alappat@ehealthce.ca

Conference - U of T Scarborough & Scarborough Health Network

U of T Scarborough (UTSC) and SHN invites you to a two-day conference, which will bring together presenters with clinical and research expertise, scholars and community perspectives. Hosted at UTSC on October 13 and 14, the Building Communities through Inclusive Health conference aims to address the root causes and consequences of health inequities, the role of communities in promoting health equity, and policies and programs to support community solutions. The conference is free to participants. Register today.

Ontario Lab Information System (OLIS)

The Ontario Laboratories Information System (OLIS) is an information repository that gives authorized health care providers access to lab test orders and results from hospitals, community labs and public health labs. 

The eHealth Centre of Excellence, OntarioMD and Ontario Health are collaborating on an eOrdering Limited Production Release project to connect your EMR to Ontario Lab Information System (OLIS) through the mobile orders and results entry web form (OLIS-MORE). The eOrdering workflow will support ordering of all labs (including public health). Lifelabs, Alpha Labs and Dynacare are participating.
