Welcome Note from Scarborough Family Physicians Network (SFPN) Co-Chairs

Hello Fellow SFPN Members!

We would like to thank you for your tireless efforts in caring for your patients in these difficult and changing times. The Scarborough Family Physicians Network leadership team continues to work in the background and on your behalf to contribute to the groundwork for our evolving Scarborough Ontario Health Team (OHT). Your hardships in managing COVID and your day-to-day practice are on our radar as we meet with our OHT partners to develop programs for improved patient care, system navigation and technology integration. Together, united, we will move the needle forward in our Scarborough medical community.

We are pleased to present the SFPN inaugural newsletter. This will periodically bring news and updates about what SFPN is doing on your behalf, and provide invaluable resources for your practice. 

We need your input!  Got concerns and pain points related to your practice?  Please let us know. While we can’t promise to solve all problems, your input will guide our OHT work and help focus our efforts in finding solutions.

The Scarborough Family Physicians Network is pleased to announce their 2022-2023 strategy. The strategy is set to focus on foundation priorities for building the network, and tactical priorities focusing on addressing the needs and challenges of family physicians in Scarborough.

Online Appointment Booking

In recent developments, primary care providers are encouraged to provide online appointment booking (OAB) for their patients. SFPN and SOHT are providing implementation support for OAB to providers working in primary care. Our staff have the expertise to support you at NO cost to you. Licensing cost will be covered up to March 31, 2023. If you are interested in using online booking, please complete this form.

SCOPE

Seamless Care Optimizing the Patient Experience (SCOPE) allows community physicians to quickly and easily connect with an Internist or a Nurse Navigator. By calling one phone number, PCPs have access to phone consultations with an Internist at Scarborough Health Network (SHN) for advice or to refer a patient for urgent work-up and assessment. In addition they also have access to a nurse navigator, who supports their access to over 40 community resources and programs such as mental health, palliative care, adult-day, youth programs and more.

Ocean eReferral

SHN, SOHT, and SFPN are working together to support healthcare providers in our community who may be interested in onboarding onto Ocean and/or EpicCare Link.                 
Community providers and providers affiliated with SHN who are interested in being onboarded to Ocean, can register here. For additional information or to request a demo, please email your Ontario Health East Deployment Team.
