For vaccination needs or for general questions, Visit the resources page on the SFPN or SOHT Website for more information. ![]() One Stop Talk: A new mental health supportOne Stop Talk/Parlons maintenant (OST/PM) is a new virtual and confidential counselling service rolling out across the province. It provides children and youth (aged 0-17 years) with immediate, low barrier access to free mental health support when needed.When a child or youth connects with One Stop Talk/Parlons maintenant through www.onestoptalk.ca, or 1-855-416-8255, they will be connected to a professional clinician for a virtual or telephone session of brief therapy. OST/PM aims to fill the service gaps that leave children and youth waiting and struggling to navigate a complex healthcare system while increasing the availability of in-the-moment intervention.![]() Scarborough SCOPE (Seamless Care Optimizing the Patient Experience)Scarborough Health Network (SHN) and Scarborough Centre for Healthy Communities (SCHC) have been working hard to expand and improve the Scarborough SCOPE (Seamless Care Optimizing the Patient Experience) service that our organizations have partnered on to provide. We invite primary care providers (PCP) to get started with SCOPE today. To register, please complete the provincial SCOPE online registration form. Our virtual, inter-professional care team connects primary care providers to specialists, hospitals, community care and mental health resources in real-time. If you are not currently using SCOPE, click below to learn more and register. ![]() SHN's Centralized Patient Scheduling (CPS)SHN is transforming the way patients and families access services at Scarborough Health Network (SHN) by Centralized Patient Scheduling (CPS). With the new CPS model, patients will have a more convenient way to book, reschedule, or cancel their appointments through a single point of contact. This will be facilitated through the centralized office with dedicated scheduling agents and extended service hours, including evenings and weekends. A visual overview is attached to this email, to outline what this service transformation means for you and your patients. The aim is to provide integrated and comprehensive care to individuals diagnosed with schizophrenia, promoting their well-being and facilitating their successful reintegration into the community. Our vision is to establish a seamless and supportive care pathway that enhances the quality of life for all participants.
