SFPN Call out for Board Nominations - 2024

Make a Difference at Scarborough Family Physician Network (SFPN) 

As we approach the Annual General Meeting of Scarborough Family Physician Network (SFPN) on May 29th, 2024, we are looking to fill open director positions on the SFPN board. We are inviting all interested members to submit nominations.

As a member of the SFPN, this is your opportunity to be the voice of primary care as we collaborate with our healthcare partners to improve patient care and provider experience, increase efficiency in healthcare delivery, and build a stronger community in Scarborough. 

If you are interested, we encourage you to submit a nomination form. If you know someone who you believe would make an excellent director, please ask them to submit a nomination form. The deadline for nominations is April 7th, 2024 at 5 pm.

SHN Orthopedics Program

SHN’s orthopedics program has the largest complement of orthopaedic surgeons in Ontario and performs more than 5,000 surgeries across every orthopaedic subspecialty. While wait times for total hip and knee arthroplasty remain problematic across the province, SHN is well positioned to address the needs of this surgical population.

We provide rapid access to care through our Orthopaedic Navigator, which has resulted in some of the lowest wait times for total joint replacement in Ontario. On average, patients wait less than three weeks for their elected procedure from the moment the decision to treat has been made.

Our SHN Orthopaedics program can provide patients with rapid access for total joint replacements. To refer a patient, please connect with our Orthopaedic Arthroplasty Team.

Mental Health & Addictions at SHN

Scarborough Health Network provides mental health care services to meet the unique needs of individuals experiencing mental illness. 

These include inpatient and crisis services, as well as outpatient programs that help patients transition with greater ease from hospital to the community and to reduce the stigma often associated with mental illness. We also have support and social recreational programming for individuals and families affected by severe and persistent mental illness.

SFPN  & SHN's Mental Health & Addictions program have partnered together to address concerns by primary care providers about access, communications, etc.. As a result, SHN has committed to providing timely updates to keep SFPN members apprised of what to expect when they refer patients to the services and how to obtain access to more rapid services when needed.

New SHN Urgent Care Centre at Centenary Hospital

SHN has launched a new Urgent Care Centre at our Centenary Hospital, located in the same space where previously there was a private after-hours clinic.

The Urgent Care Centre offers patients walk-in access for non-life-threatening conditions, including: fevers, coughs and colds, infections, nose and throat complaints, earaches, eye injuries, sports injuries, deep cuts, minor burns, insect bites, allergies, and rashes. They also provide point-of-care testing, casting services, treatment for minor lacerations and wounds, as well as prescriptions and requisitions.

Online booking will soon be available.

Location: Centenary Hospital, Medical Mall, 2nd Floor (near the Shoppers as you arrive through the main North entrance).

Hours: Monday to Friday, 3 – 7 p.m. and Sunday, 1 – 5 p.m.

Ocean eReferral updates

Several new sites are now actively accepting referrals via Ocean:

  • Women’s College Hospital (WCH) is now accepting eReferrals for its Endocrinology Clinic and Gynecology Program
  • For a complete list of WCH services live on eReferral, search “Women’s College” on the Ocean Healthmap.
  • KMH Cardiology Centres provide Cardiology, Nuclear Medicine, MRI, PET/CT and cardiology consultations (both virtually and in-person); they are now accepting eReferrals at their Toronto, North York, and Scarborough locations.
  • MGH continues to accept eReferrals for the majority of its services, including the  Endocrinology Clinic, Cardiology, Heart Function and Pacemaker Clinics, as well as for Diagnostic Imaging (x-ray, ultrasound, mammography, nuclear medicine, bone density scans, CT, and MRI)

SCOPE

Scarborough’s expanded SCOPE program offers primary care providers a single point of access to navigate hospital and community services for your patients.

Available services include: Nurse navigator, internal medicine, mental health, diagnostic imaging and fracture clinic.

SHN Centralized Patient Scheduling Wave 2

We are excited to share our second wave of clinics transitioning to the CPS model will begin next month, rolling out program by program until June.

Wave 2 CPS Go-Live Schedule:

  • March: Cardiovascular and Stroke (Cardiac Diagnostics, EEG, EMG)
  • April: Nephrology, Diabetes, and Child and Adolescent Mental Health
  • May: Adult Mental Health
  • June: Surgery

The CPS model gives patients a more convenient way to inquire about their appointments through a single point of contact (by calling 416-431-8167), through the centralized office with dedicated scheduling agents and extended service hours, including evenings and weekends. Measles Public Health Update

Measles is a highly contagious respiratory virus that causes a febrile rash illness and poses significant
health risks.

Individuals travelling to destinations outside of Canada should ensure they are adequately vaccinated
against measles prior to travel.
